All coaches must complete the registration form found here. After completing the online registration form, download and complete the documents below.
Completed forms must be emailed to Adrien@zumwaltcpa.com. Please cc SARS administrator, Jason Welker (jason@sars.net). Add the subject line “SARS employee paperwork”
NOTE: All employees should fill out new paperwork if they have not been employed by SARS in the last 90 days.
- Please also provide a copy of your drivers license and a voiced check so the team can set up direct deposit.
- Download the forms below, print and scan or complete on your computer, and send to the email addresses above.
- Alternatively, you may drop your forms off directly at the accountant’s office at 1224 Washington Ave Suite 201, Sandpoint
