All coaches must complete the registration form found here. After completing the online registration form, download and complete the documents below.
Completed forms must be emailed to Jamie at jamie@sars.net. Add the subject line “SARS employee paperwork”
- Please also provide a copy of your drivers license and a voiced check so the team can set up direct deposit.
- Download the forms below, print and scan or complete on your computer, and send to the email addresses above.
- Alternatively, you may drop your forms off directly at the Clubhouse at Schweitzer
